Firstly, we need to collect details for your HMRC codes. We then electronically request your authority via the HMRC website in order that we can represent you when dealing with the HMRC. This replaces the old paper-based form 64-8. You will receive a code from the HMRC, please send this code to us immediately so we can complete the authorisation process. They will send you the code by letter to the address they hold on file for you.
In accordance with Anti Money Laundering regulations, we are required to request proof of identification from all clients.
These details are required so that we can setup your direct debit schedule and billing as agreed.

How is your data is collected and stored?
The data submitted on these forms is collected securely via Cognito Forms. As soon as we have received the information the data is permanently deleted from Cognito. Your data only exists on our systems.
